Kornea naklinin tarihçesinde, 1886 yılında gerçekleştirilen ilk başarılı lamellar keratoplasti ile 1905 yılında gerçekleştirilen ilk başarılı penetran keratoplastinin örneklediği tekniklerin rekabeti önemli bir yer tutar. İmmün reaksiyonların bilinmediği o yıllarda cerrahi başarısızlıklar tümüyle teknik yaklaşımın rolü ve etkisi olarak ele alınmış, rekabet uzun bir zaman devam etmiştir. İmmünosüpresif ilaçların, mikrocerrahi sütürlerinin ve ameliyat mikroskobunun icat edilmesi ile penetran keratoplasti altın çağını yaşamış, 1995 sonrasında ise lamellar cerrahi konusundaki farkındalık yeniden oluşmuştur. Bu derleme yazısında, stromal kornea hastalıklarında neden ilk tercihin ön lamellar keratoplasti olması gerektiği, cerrahi hedefe ulaşılamadığında ise sığınılacak güvenli limanın neden penetran keratoplasti olduğuna dair literatür bilgileri ve klinik tecrübelerimiz paylaşılmıştır.

Throughout the history of corneal transplantation, there has been an important competition between the precedent-setting techniques lamellar keratoplasty, which was first successfully performed in 1886, and penetrating keratoplasty, which was first successfully performed in 1905. As immune reactions were unknown at that time, surgical success was attributed entirely to the technical approach, and this rivalry persisted for many years. Penetrating keratoplasty saw the golden age with the introduction of immunosuppressive drugs, microsurgical sutures, and the surgical microscope, but awareness of lamellar surgery began to reemerge after 1995. In this review, we discuss relevant literature and share our clinical experiences regarding why anterior lamellar keratoplasty should be the first choice in patients with stromal corneal diseases, and why penetrating keratoplasty is the safe harbor to return to when surgical goals cannot be achieved.
