Endotelyal Keratoplastide Güncel Teknikler

Özet
Endotelyal keratoplasti, 2000’li yılların başından beri kornea endotel disfonksiyonu bulunan hastaların tedavisinde tercih edilen bir yöntem olmuştur. Penetran keratoplastiye göre daha hızlı iyileşme, daha az intraoperatif ve postoperatif komplikasyonlara neden olmaktadır. Görmeyi ciddi düzeyde azaltan korneal skarı bulunan hastalarda tercih edilecek bir yöntem değildir. Değişik tipte endotelyal keratoplasti yöntemleri: Descemet soymalı (otomatize) endotelyal keratoplasti, Ultra-thin Descemet soymalı otomatize endotelyal keratoplasti ve Descemet otomatize endotelyal keratoplastidir. Descemet soymalı endotelyal keratoplasti en sık tercih edilen endotelyal keratoplasti tekniğidir. Donör korneada reziduel stromanın bulunduğu endotelyal keratoplasti tekniklerinin, Descemet membran endotelyal keratoplasti donör dokusunun manipülasyon zorluğundan dolayı zor vakalarda uygulanması daha uygun olabilir.

Current Techniques in Endothelial Keratoplasty
Abstract
Since its introduction in the early 2000s, endothelial keratoplasty has quickly become the treatment of choice for patients with corneal endothelial dysfunction. Endothelial keratoplasty allows for faster visual recovery and decreased intraoperative and postoperative risks compared to penetrating keratoplasty. Endothelial keratoplasty is not an appropriate keratoplasty option for patients with significant vision-limiting corneal scarring. There are several types of endothelial keratoplasty procedures: Descemet stripping automated endothelial keratoplasty, Ultra-thin Descemet stripping automated endothelial keratoplasty and Descemet membrane (automated) endothelial keratoplasty. Descemet stripping endothelial keratoplasty is the most commonly performed endothelial keratoplasty procedure. Procedures using donor tissue with some residual stromal tissue may be more appropriate for more challenging cases given the difficulty of handling Descemet membrane endothelial keratoplasty donor tissue.
