Rektus Kas Kopmalarında Tedavi Yaklaşımı: Bir Olgu

ÖZET
Göz dışı kaslarında kopma nadiren görülür ve tedavi yaklaşımı acil ve dikkatli eksplorasyon gerektirir. Bu yazıda iç rektus kasının insersiyosunun 10 mm uzağından travmaya bağlı olarak kopma gelişen olgunun tedavi yaklaşımı sunulmaktadır. Kopan kasın proksimal ve distal parçaları bulunarak direkt anastomoz ile sütüre edildi ve aynı taraftaki lateral rektus kasına Botulinum toksin enjeksiyonu uygulandı. Postoperatif 1. günden itibaren primer pozisyonda ortoforik olan hastanın bulgularında 1 yıl süresince değişiklik gözlenmedi. Bu olguların tedavi seçenekleri ve prognozları literatür eşliğinde tartışıldı.

Management of Rupture of Rectus Muscle: Case Report
ABSTRACT
Rupture of the extraoculer muscle rarely occurs and its management requires early and careful exploration. We report the management of a case with a rupture of the medial rectus muscle occuring more than 10 mm from the insertion secondary to trauma. Proximal and distal segment of the ruptured muscle was found and sutured with direct anastomosis and intraoperative Botulinum toxin was injected into the ipsilateral lateral rectus muscle. Primary position orthophoria was achieved in the first postoperative day and it was maintained for the 1-year follow-up. Treatment options and prognosis of these cases were discussed with the review of current literature.
