Aort Diseksiyonu ve Bilateral Anterior İskemik Optik Nöropati

ÖZET
Elli yaşındaki erkek hasta acil servise 1 hafta önce başlayan sol gözde ani görme kaybı ile başvurdu. Hastanın yapılan oftalmolojik muayenesinde sağ gözde geçirilmiş anterior iskemik optik nöropatiye bağlı optik atrofi, sol gözde anterior iskemik optik nöropati saptandı. Boyun ve toraks bilgisayarlı tomografi (BT) ve BT-anjiyografisi ile arkus aorta distal kesiminden itibaren başlayan ve inen aorta boyunca devam eden aort diseksiyonu tespit edildi. Her iki internal ve eksternal karotis arter doğal olarak izlendi. İskemik optik nöropati orta ve ileri yaş grubunda ciddi görme azlığı ve körlüğün başlıca nedenlerindendir. Bizim olgumuzda predispozan risk faktörü olarak hipertansiyon ve eşlik eden DeBakey tip 3b aort diseksiyonu varlığı literatürde daha önce tariflenmemiştir.

Aortic Dissection and Bilateral Anterior Ischemic Optic Neuropathy
ABSTRACT
A 50 -year-old male patient admitted to the emergency service with sudden loss of vision in the left eye which started one week ago. Ophthalmological examination revealed optic atrophy because of ischemic optic neuropathy in the right eye, ischemic optic neuropathy in the left eye. Aortic dissection starting from the distal part of the aortic arch and continuing along the descending aorta was idetified with neck and chest computerized tomography (CT) and CT-angiograpy. Both internal and external carotid arteries was considered natural. Ischemic optic neuropathy is a major cause of severe vision loss and blindness in middle and older age groups. In our case, predisposing risk factors such as hypertension and concomitant presence of DeBakey type 3b aortic dissection has not been described previously in the literature.
